This poem is very easy to relate to. :)

Time to say goodbye,
time to let go of you for good,

A very strong start. The reader knows right from the start the struggle you're going through.

the reality of us being apart
finally hits all at once.

The shock of breaking up is hard to deal with, but when the pain finally comes.. it's horrible. It just you in the chest like a brick.

stomach painfully twisting,

I've dealt with those painful stomachaches. I thought it wasn't really real until it happened to me. I didn't really think breaking up with someone could really make your stomach actually hurt. I just thought it was all in their head. But I'm a believer now.

reminding myself you aren't worth it,
the right guy won't give up.

This is a great line, not only because it helps you remember he isn't worth is, and the right guy won't give up, but because it reminds the readers that the right guy won't give up on their relationship. So it's very inspirational. :]

How can I let go of you?

I can feel the frustration and sadness in this line. You don't know how you're going to actually let him go.. and you don't even really want to.

Please, wake up, remember our love,
till the day comes where I walk away
never to turn back around,
till the day I don't shed a single tear.

You still have hope for your relationship. It's only a little hope, but it's still there. You don't want to lose him forever. You just want him to realize what he's doing and stop. You want him to save the relationship before it's too late.

It looks like you've finally come to terms that you can't stop what is happening, so you just need to say good-bye, but you still don't want to.

I love the ending. It leaves the reader wondering if you said good-bye or not. I love those king of endings. :]

Love is an evil thing that brings our deepest emotions out whether it be happiness or sadness. But whichever it is love is a very powerful thing. I think you portrayed your emotions beautifully in this. And I bet it felt nice to vent. :]
