My fault

Every opportunity Ive been given
Has just slipped me by
Every chance that could be taken
Ive just said goodbye

Its my fault

But tomorrows another day

Another day to try to find
That something that Ive left behind
Another day, another lie
Another chance to slip me by

    The first step to fixing your mistakes is admitting that they exist, which you have b/c now you can be conscience of it, and do something about it, make a change. Your poem expresses a lot, good job with it.
