Awww-sweet poem! I love the rhyme; It didn't feel forced at all--The flow is flawless and your word choice is perfect-- My favorite part is the last few lines of the last stanza:

"That love is Love,
And extravagantly true,
This is where you realize,
Love was made for you."

The fact that you capitalized love in this first line of this section definately made your message more powerful and the last line sums up the poem very nicely!

Now here you use the words that show us we are reading an illusion but this one is comforting and begs the reader to understand the message and for a minute take part in it.

Awww! That is so sweet, it's amazing! I love it, it places me in "Awe" I really loved the lines: "Look at the Heavens,
Look at the stars,
This is where,
Love gently falls."

That was so beautiful, and great, and so lovingly felt. :]
