I thought this was a very beautiful poem. It was short, sweet, sad, and simple. You did a great job capturing the emotions in this piece. The only thing I have to suggest is making your poem more professional looking. For example:

"Do you realize how much you mean to me?
Maybe one day you'll finally see....
I love you.
My only wish before i go,
Is that you say it too."
