anyone else have this problem?? PLZ HELP!!!

this is kind of hard for me to explain, but i'll try my best...

for the past few years, i've had this problem where for days, sometimes weeks at a time i go without talking. if someone tries to talk to me or ask me questions, i will answer them, and i think it's out loud, but it turns out that i'm only talking in my head and i don't realise it. i cant figure out how to get myself to actually talk. i never realise it until people keep repeating things or start yelling at me. does anyone else have this problem or know how to help, because this is getting me really frustrated... all these people are thinking im going crazy and they're trying to put me back in the hospital.
