HELP ME PLEASE!!!!!!!

OK I have this teacher at my school && she was the best teacher ever but she has decided to leave us to get a better job. So some of us students are putting together a memory book for her && we are looking for a poem to thank her for all the help she has given us. Can anyone help me?????
